All health plans offered on the Health Insurance Marketplace include certain items and services.

What benefits make up your new comprehensive health plan?
EHB must include items and services within at least the following 10 categories: ambulatory patient services, such as doctor visits; hospitalization; mental health and substance use disorder services, including behavioral health treatment; rehabilitative and habilitative services and devices; laboratory services; emergency services; maternity and newborn care; prescription drugs; preventive and wellness services and chronic disease management; and pediatric services (pediatric oral care may be included, or in the Marketplace, offered as part of a stand-alone dental plan).
Remember, grandfathered health plans are exempted from many changes required under the Affordable Care Act, including the requirement to offer EHB. Also, specific health care benefits may vary by state.

Use this table to find out whether your income and household size will make you eligible for a subsidy in 2019. (These numbers are based on the 2018 federal poverty guidelines. Since open enrollment for 2019 will take place in 2018, these numbers will be used for all plans with 2019 effective dates.) For instance, as the table indicates, a family of three with household income up to $83,120, and a family of five with income up to $117,680, are eligible to receive a premium tax credit for 2019, depending on the cost of a Silver plan in their area.

The subsidy will make up the difference between the amount an individual is expected to contribute (based on income) and the actual cost of the area’s second-lowest-cost Silver plan.

For most families it is yourself plus the number of people that you claim as dependents on your income tax return. This may include children, parents, or other relatives who qualify as dependents on your tax return. Children of divorced parents are counted as the family of the parent who claims them as a dependent (even if the other parent has to pay for the child's health insurance). Do not include children who earn enough to support themselves, and so are no longer eligible as dependents, even if they still live at home.
Expected 2014 gross income (before taxes) including wages, tips, net profit from self-employment, interest, rental income, and other investment income, most pensions, social security payments and alimony. This will be the amount called "Modified Adjusted Gross Income" shown on your tax return in line 4 of Form 1040EZ, line 21 of 1040A or line 37 of form 1040. If your only income is from a job it is the number shown in box 1 of your W2 form. Include income of all dependents (for example a child's summer earnings or dependent's social security) even if they filed a separate tax return.
